GRANT APPLICATION
Purpose

Greater St. Louis Health Foundation will consider funding applications for amounts up to $100,000 for projects that address the Foundation’s purpose, which is:

"To promote health and prevent illness in the Greater St. Louis area through support and financial aid to not-for-profit organizations."

Review Cycle

Applications will be approved by the Foundation’s Board of Directors which meets in February, May, August and November. Applications must be received by the Secretary no later than the last day of the months of February, May, August or November to be considered at the next Board of Directors meeting.

During the review period, the Foundation may request additional information, require a personal presentation by members of the applying organization, or ask to make a site visit to the applicant facilities or other location(s) relevant to the application.

Applicants will be notified of the disposition of their applications as soon as possible and, in most cases, within two weeks of the Board meeting at which their application was considered.

Grant Application Procedures

Two copies of your application should be addressed in the form of a letter to the Secretary of the Foundation. Letters of Application will:

  1. Describe the intended use of funds, showing how these match the purposes of the Greater St. Louis Health Foundation;
  2. Describe specific project goals and objectives and how these are to be achieved and measured;
  3. Describe the organization of the project and qualifications of project staff;
  4. Provide a project timetable or milestone chart, as well as a description of how results will be evaluated (may be included as an attachment);
  5. Provide a project budget (may be included as an attachment);
  6. Describe, if applicable, how the project will sustain itself once Foundation grant funds are exhausted;
  7. Attach a copy of the applying organization’s articles of incorporation and bylaws, as well as proof of not-for-profit status.

Multiple Applications
The Greater St. Louis Health Foundation must be apprised of all applications pending or intended for submittal to other foundations or government agencies.
Awards

Distribution and scheduling of fund draw-down will be negotiated based on reporting timetables established by the applicant.

Awards are made for a period of one year only and are not renewable.

Awards are not granted to individuals, or for the satisfaction of graduate or professional degree requirements.

The Foundation will not fund projects outside of the Greater St. Louis area.

In general, funds are not awarded for capital campaigns, general organizational expenses, or for "bricks and mortar."

Should the project be discontinued for any reason, unused funds will be returned to the Foundation. Should duplicate funds be received from any source, all funds received from the Greater St. Louis Health Foundation must be returned. This requirement may be waived if justification is provided in the application.

In some instances, the Foundation may award only a percentage of the requested amount. If this is unacceptable or materially affects the accomplishment of the project, please so state in the letter of application. All applications should state the minimal amount acceptable.

Audit

It is the applicant’s responsibility to provide the Foundation with a project financial audit at the conclusion of the project.

Final Report

It is the responsibility of the successful applicant to provide the Foundation with a complete report in writing at the conclusion of the funding period.
